The popularity of GLP-1 receptor agonists—such as semaglutide, marketed under brand names like Ozempic® and Wegovy®—has surged in recent years, both for the treatment of type 2 diabetes and as an aid in weight loss. While these medications offer significant benefits, they are also associated with gastrointestinal side effects, including nausea, vomiting, and delayed gastric emptying.
Of particular concern in the surgical setting is the risk that delayed gastric emptying may increase the likelihood of regurgitation and aspiration of stomach contents during general anesthesia or deep sedation. This can pose serious risks, especially when patients are presumed to have an empty stomach.
Because semaglutide has a long half-life—approximately one week—the American Society of Anesthesiologists (ASA) currently recommends that patients discontinue GLP-1 agonists at least one week prior to elective procedures to help reduce these risks.
For urgent or emergency surgeries, where discontinuation is not possible, the ASA advises that the anesthesia team follow "full stomach" precautions to enhance patient safety during sedation or anesthesia.
